ONLY THOSE APPLICANTS WHO MEET THE ELIGIBILITY REQUIREMENTS WILL RECEIVE AN EMAIL ON OR ABOUT DECEMBER 12 OR 15 WITH AN APPOINTMENT DATE AND TIME. THIS APPOINTMENT CANNOT BE CHANGED AND THE APPLICANT MUST ATTEND IN PERSON.

The following documentation must be submitted to SALARMYFREEPORT@GMAIL.COM within 24 hours of submitting your application or your application will be rejected. You can scan it or take a picture and add it to the email.
• Power bill number (not older than 3 months)
• NIB card for every adult over the age of 18 who is living in the household
• If working, upload income information showing $ amount
• Income information needs to be provided for each member in the household who is working
NOTE that all related persons living at the same address are considered to be one family.
